family as a whole."
At-Home
At-Home provides early intervention assisting families in navigating
the challenges of risky behavior of the adolescent child and stresses
of home-life. The At-Home effort concentrates on helping the family
generate the necessary support, engage community resources alongside
the family, and guides the parents on how to best minimize an out-of-home
experience.
A Transition Specialist, similar to an intensive
case manager, visits the home to gain perspectives of the familydynamics
and interview its members. Through our assessment tools, we will identify
as well as in the coordination of necessary services that support and
make the most of family and community resources.
By addressing the issues sooner than later and gaining professional
support immediately the family can:
- Minimize the disruptions to family life, spare traumatic and hurt
feelings, and minimize resentments.
- Minimize the financial burden with a preventive and early intervention
approach.
- If an out-of-home placement becomes necessary,
we guide the family to educational consultants for placement and
provide support to the family through the decision process, enrollment
and aftercare. We
work closely with program professionals to achieve highest levels
of results and provide continuity of care.
As a result, the family concentrates on leveraging
resources to develop and stabilize its own internal structures, limiting
the family’s
need for future outside interventions. |
